Registration Form

Word Fill-in Form

	Cost per player is $120.00 per week
	Family discount: $80.00 for each additional sibling. If a child attends more than one session, than the cost of the additional sessions is $80.
	Team discount: $100.00 per player for teams with at least 8 players.

If one child attends two camps, the first week is $120.00 and the second week is $80.00.

If two children attend two weeks the cost is $120 + $80 + $80 + $80 = $360
